In a matter to define your approach of outpayment, please deliver the following points to your attendance:
- Proof of Identification
- Proof of address
- Proof of income or Medicaid/Health Insurance/Medicare
The Women’s Health Program also demands:
- Social Security Card
- Birth Certificate or Passport
For Prenatal Care we demand proof of pregnancy. If necessary, we will supply a pregnancy test at the time of attendance
If you do not deliver these papers, you will have to pay the total charge or be re-registered.

Male Physicals
Thursday 9:00 a.m. – 3:00 p.m
- Walk-in painless urine STD testing accessible at any time
- Treatment for Venereal Warts or other STDs
