In order to determine your method of payment, please bring the following items to your appointment:
- Proof of Identification
- Proof of address
- Proof of income or Medicaid/Health Insurance/Medicare
The Women’s Health Program also requires:
- Social Security Card
- Birth Certificate or Passport
For Prenatal Care we need proof of pregnancy. If needed, we will provide a pregnancy test at the time of visit
If you do not bring these documents, you will have to pay the full fee or be rescheduled.

- Treatment of sexually transmitted diseases and infections
- Family planning and birth control options (Implanon, IUD’s, Nuvaring, Patch, etc.)
- Diagnosis and treatment for irregular menstrual cycles
- Menopause treatment/Alternative therapy (Bio-identical Hormones)
Male Physicals
Thursday 9:00 a.m. – 3:00 p.m
- Walk-in painless urine STD testing available at any time
- Treatment for Venereal Warts or other STDs
